For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. Consent to Proxy Access for Online Services
The Proxy online access form should be completed if you wish to apply for access to online services for someone you care for including children. Once you have received the Patient Access registration information sheet from the surgery please Click here to log onto Patient Access. You will then be able to enter your personal details and password as shown on the Patient access registration information sheet. You can use any of the services listed below:

This feature allows you to request repeat prescriptions from your own home. Without visiting the practice, you can use the internet to quickly order any repeat prescriptions that are due.
Once ordered simply collect your prescription as normal, 48 hours later.
Change of address enables you to add or update your address and contact details, including home and mobile numbers and email address.
Changes made through this service are subject to approval by the practice.
Patient Access users can now view their Medical Record. You must complete an application form to access this service and provide photo ID. Access is based on practice approval. If you already have log-in details and wish to view your medical record please ask at reception for an application form.
