Who do I see?
Routine appointments
We’re changing the way our appointment system works.
Starting from Tuesday 2.4.24, all on the day GP appointments will now be available in the morning from 8am.
From 8/7/2024 Pre-bookable appointments will be available to be booked from 8am for the following week too.
You no longer need to wait for the afternoon appointments to be released at 2pm.
You can access these appointments by calling us when the appointments are released, or use the NHS app or online services to book the appointments.
Please also make sure you book a separate appointment for each person needing attention. One Appointment One Problem. For more than one appointment you will be asked to book another appointment.
You may be offered an appointment with one of our partner organisations who are offering appointments on behalf Church Elm Lane Medical Practice and this may be at another site nearby.
Appointments with Nurses, Clinical Pharmacists, Healthcare Assistant and Physiotherapists
All appointments with Nurses, Clinical Pharmacists, Healthcare Assistants and Physiotherapists are bookable in advance up to 4 weeks. Please check with our reception team if you wish to book an appointment.
Cancelling your appointment
Patients are advised to cancel their GP appointment with 1 hour in advance notice and Nurse/Pharmacist/HCA 24 hours in advance of their appointment. Your appointment will be classed as DNA appointment if you do not cancel within these timeframes.
If you arrive 10 minutes after your appointment time without contacting the practice beforehand, please be advised that you may not be seen. Please be aware that this is the GP decision whether they accept to see you if you are late for your appointment.
Please note if you do not attend 3 appointments you may be removed from the practice register.
Consult with a Doctor online
E-Consult is a new method of getting the right help. You can use the online e consult portal to self check symptoms and receive on the spot medical advice 24/7. Please visit the E-Consult website for further information.

If you need an urgent appointment, contact the practice during opening hours between Monday to Friday 8am to 6.30pm on 020 3770 1888
When the practice is closed, please ring NHS 111 or use NHS 111 online and they can help you with booking an urgent GP appointment.
Routine and urgent appointments are provided in the evening and weekends at the following locations. You will be told which location you need to travel to when your appointment is booked:
- Barking Hospital Hub – Upney Ln, Barking IG11 9LX
- Becontree Hub – 641-645 Becontree Ave, RM8 3HP
- Broad Street Hub – Morland Rd, Dagenham RM10 9HU
- Parsloes Hub – 2 Hobart Rd, Dagenham RM9 5NH
You should only attend A&E if you need immediate care for something very serious or life-threatening. If your condition isn’t life-threatening, in many cases, you could be seen faster by using other local services in your community.
If you feel unwell or have a minor injury, your local pharmacy can also offer advice and some medicines. Many pharmacies are open late and at the weekends, and you do not need an appointment to be seen, Find your local Pharmacy
Home Visits
Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception before 10:30.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls
You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
